MedPay

MedPay is an option for most auto insurance carriers. It will pay for treatment without affecting premiums. They recoup the money from the party-at-fault's insurance company so there is no expense to them.

MedPay is the preferred way for most doctor's offices to receive payment as the party-at-fault's insurance disburses payment after the case is settled, which may take years.

What to bring:

• Insurance Information
• Claim Number
• Police/Accident Report
• Any Emergency Room documents pertaining to incident (if applicable)
• Any X-rays, MRIs, and Radiology Reports

Personal Injury

What to bring:

• Insurance Information (Both Party at Fault and Personal Insurance)
• Claim Number
• Accident Report (if any)
• Any Emergency Room documents pertaining to incident (if applicable)
• Any X-rays, MRI, Radiology Reports

Workers Compensation

*MUST BE REPORTED TO YOUR SUPERVISOR PRIOR TO TREATMENT

What to bring:

• Employer Information (Supervisor Contact info)
• Claim Number & Date of Incident
• Any Emergency Room documents pertaining to incident (if applicable)
• Any X-rays, MRI, Radiology Reports
